Talking in chance, what does the term "Once in a Blue Moon" mean?

This actually has got nothing to do with the colour of the moon. It does mean "very rarely," but it is referring to the occurrence of there being two full moons in one calender month. As the full moon happens once every four weeks, then twice in 30 or 31 days is a rare event.
